Brooklyn Podiatrist - New Visit - Patient Forms
The following documents will assist you in expediting the check in process for your appointment. We would ask that you please print and complete the following forms and bring them with you to your appointment. Thank you for your cooperation.

Instructions:
Click on the link below to access the New Patient Form. After you click the link, a browser window will open. After it opens, click on the 'PRINT' icon on your Browser. After the form prints, fill it out and bring it into our office. Completing the New Patient Information Form will save you time on your initial visit!
What else you should bring…
- Your insurance card along with another form of I.D. (i.e. – Driver’s License, State ID card, Military ID)
- Previously X-Rays, MRIs or other pertinent medical records, if you have any
- Information about prescription medication which you may be taking
- Cash or Checks for your co-pay
